If I have to cancel an appointment what should I do?
If you need to cancel or reschedule your appointment, please give us at least 24-hours advance notice.

Are there any forms to fill out once I arrive?
After you make your appointment then fill out our Online Registration form. This will give you the opportunity to fill out the form in the privacy of your own home, and give you quick access to your medical records. By filling out the form before your appointment you do not have to arrive 15 minutes early to fill out it in our office.

How can I get my prescriptions refilled?
You can either have your pharmacy call them in or contact the office for more information.

What insurance do you take?
We are providers for many managed care plans and health maintenance organizations and, for your convenience, we gladly file your insurance claims for you. If you do not find Dr. Rosenfeld in your Provider Directory please call as these plans change daily.

Can I pay with cash or credit card?
We take Visa, MasterCard, Discover, AMEX and checks.

I'm pregnant, what happens next?
